To replace a lost, stolen or damaged Florida title, submit HSMV 82101, Application for Duplicate or Lost in Transit Title, at your local tax collector office. A duplicate costs $75.25, plus $2.50 if a paper title is printed. Some counties offer a fast title for $10 more.
What you need
- HSMV 82101, completed and signed by the owner(s)
- The damaged title, if it was damaged
- Address verification, if your address differs from DMV records
- Lienholder details (or “None”)
- The fee
Filling in HSMV 82101
It’s a fillable PDF from FLHSMV.
- Section 1, type of application. Tick Duplicate and say whether the title was lost, stolen or damaged. Tick Lost in Transit instead if the title was issued within the last 180 days and lost in the mail: then there’s no fee.
- Owner details. Name(s), email, mailing address and date of birth.
- Section 2. Who is applying: owner, lienholder or dealer.
- Section 3, vehicle. VIN, make, year, body, color, plate and Florida title number.
- Section 4, usage/brands. Tick any that apply (lease, rebuilt, flood and so on).
- Section 5, lienholder. If there’s no lien, print “None”. If there is, the title goes to the first lienholder unless they authorize sending it to you.
- Section 6, odometer and signatures. Write the current reading (no tenths), tick whether it’s actual mileage, then sign.
Selling at the same time
HSMV 82101 has a “Duplicate with Transfer” option. Both seller and buyer must be present, and the buyer’s details go in the purchaser fields. This lets you sell without first waiting for the duplicate.
Submitting it
Take it to a tax collector office or license plate agency (find one). Expedited “fast title” service is available in some locations for $10.
